Patient Financial Information

Please bring your current insurance, Medicare, or public assistance information with you on the day of your procedure. If required by your insurance provider, obtain preauthorizations, physician referral, or second opinion documentation prior to your appointment.

Facility Fees

Prime Surgical Suites fees cover only the use of the facility, and do not include laboratory, pathology, surgeon, anesthesiologist or certified nurse anesthetist fees, nor does it include the cost of any implants used for your surgery. You will be billed separately for these fees.

Please let us know prior to your surgery if you need help making special financial arrangements; we’re happy to discuss alternative payment methods with you.

Collections and Payment Policy

We’re happy to accept cash, cashier’s checks, credit cards and personal checks with valid identification.

Co-pays will be due on the day of your surgery.

For patients with no insurance coverage, all fees will be due in advance.

CareCredit is a patient payment plan with eligibility determined by the patient’s out-of-pocket expense. Plans extend up to 60 months and allow up to 18 months of interest-free payments. Medicare

Prime Surgical Suites welcomes Medicare beneficiaries for treatment; we are Medicare certified and comply with all specified physical plant, staffing, safety and governance standards.

Your Rights and Protections Against Surprise Medical Bills

Prime Surgical Suites is happy to provide estimates for facility fees. Please note these estimates are based on your surgeon’s treatment plan and could vary if treatment plan changes. It does not include any patient financial responsibility for the surgeon and anesthesiologist, covered implants, or any other ancillary services or tests ordered by your doctor. You will be billed separately for these fees.
